Dietitian and Diabetes Educator

Position Description:

The Dietitian and Diabetes Educator plays a critical role in the multidisciplinary team caring for patients with chronic kidney disease (CKD), end-stage renal disease (ESRD), and other chronic/acute health issues. This position involves assessing patients' nutritional needs and developing individualized dietary plans. The professional is dedicated to helping individuals manage diabetes and other diseases through education, support, and lifestyle modifications. This role involves assessing patients' knowledge of their heath conditions, developing personalized education plans, and promoting self-management strategies to improve health outcomes.

Duties/Responsibilities:

Evaluate patients’ nutritional status, including dietary habits, biochemical data, and anthropometric measurements.
Conduct comprehensive assessments to evaluate patients’ knowledge, skills, and behaviors related to diabetes management.
Develop and implement personalized nutrition care plans that address patients’ medical conditions, treatment goals, and personal preferences. The focus is on outpatients but will support inpatients as needed.
Provide patients and their families with education about dietary restrictions, food choices, and meal planning to manage their health effectively.
Develop and deliver individualized and group education sessions on diabetes management, including medication, nutrition, exercise, and blood glucose monitoring.
Regularly monitor patients’ nutritional status and adjust dietary plans as necessary based on their progress and treatment changes.
Work closely with nephrologists, primary care providers, nurses, social workers, and other healthcare professionals to ensure comprehensive patient care.
Maintain accurate and timely records of nutritional assessments, care plans, and patient progress. Meets requirements for billing MNT and DSME.
Stay informed about the latest research in nephrology and nutrition, applying evidence-based practices in patient care.
Participate in community education programs to raise awareness about kidney health and nutrition. Participate in community education and programs to raise awareness and connect patients with appropriate community resources.
Create and distribute educational materials and resources to enhance patients’ understanding of diabetes and promote self-management.
Track patients’ progress and make necessary adjustments to education plans based on their evolving needs and health status.
Stay current with diabetes research, best practices, and guidelines to provide the highest quality of care.
Complies with all state and federal regulations and guidelines for dialysis, diabetes self-management (DSME), and medical nutritional therapy (MNT).
Other duties as assigned.

Physical Requirements:

Prolonged periods of sitting at a desk and working on a computer.
Must be able to lift up to 15 pounds at times.

Required Skills/Abilities:

Strong communication and interpersonal skills
Ability to work collaboratively in a team environment.
Proficiency in using electronic health record systems.
Must be able to pass a background check and drug screening.

Education/Experience:

Bachelor’s degree in nutrition, Dietetics, or a related field; completion of an accredited dietetic internship preferred.
Previous experience in a clinical setting, particularly with renal, diabetic, and other chronic diseases, is preferred.

Licenses/Certifications:

Registered Dietitian Nutritionist (RDN) or Licensed Registered Dietitian required in the State of North Dakota ;
Current Certified Diabetes Care and Education Specialist (CDCES) or willingness to obtain within 18 months of employment.
