Up to $15,000 Hiring Bonus for eligible rehires and external hires that meet required qualifications and conditions for payment.

Provides coordination/case management for the transplant related activities of patient caseload across the continuum of care – pre-transplant, through post-transplant. Provides patient/family education, coordinates patient evaluation, maintains waiting list registration and maintains patient readiness for deceased donor transplants. Arranges transplant admission, monitors clinical pathway progression and provides post discharge follow-up. The coordinator provides professional/peer education, and participates in the development of patient education programs. The coordinator maintains 24-hour on call coverage for organ offers, and all other transplant needs. The coordinator prepares for, and participates in outreach clinics, with physicians.

Required Qualifications:

  • Bachelor's Degree in Nursing
  • Washington Registered Nurse License
  • Remains up to date on UNOS-OPTN and CMS regulations
  • 5 years of acute care nursing experience

Preferred Qualifications:

  • Certified Clinic Transplant Coordinator (CCTC) certification
  • 3 years of current experience in cardiology nursing, including transplantation
